Birth Certificate
Live births are recorded in the Office by the applicant or his representative. Baptismal certificates of the child, or the parents; marriage contract of the parents; and voter's affidavits of the parents are required. Births not recorded within the reglamentary period are considered delayed registration. It will take ten days before delayed registration certifications are released. For those reporting out-of-town births, applicant will have to execute an affidavit to that effect, before the Office will forward the details to the Civil Registrar of the city / municipality where such birth occurred. A certificate will be issued to the Local Civil Registrar after attesting to the veracity of the documents.

Illegitimate children born before Aug. 3,1988, and if recognized by the father shall carry the surname of the father. After that date, if the father does not recognize the child, such child shall carry the surname of the mother.

Procedures for Securing a Birth Certificate:
Upon request of the applicant, the request is recorded in a logbook. While the verification of documents is processed, the applicant pays the verification fee at the Treasurer's Office. A certification is issued upon presentation of such receipt to the Local Civil Registrar's Office.

Marriage License
The applicants shall submit relevant documents (Birth Certificate, Baptismal Certificate, Community Tax Certificate) and submit to interview to ascertain veracity of civil status; and counseling seminar. Pertinent data to previous marriages, if applicable, should be declared. The age requirement of the contracting parties shall also be observed according to the guidelines of the Local Civil Registrar. Affidavits shall be executed by the executing parties, whenever required, before the marriage license is issued.

Death Certificate
Upon presentation of proper certification of death by the barangay captain an / or rural health physician, the Local Civil Registrar, issues a death certificate.

For late declaration of death, proper affidavits will be secured by the applicant, before death certificates are issued.
